1x2 optical switch made by Luminos for a large biotech firm.  These have OEM part number ILC012 and are very similar to Luminos CO12-M4-FCU-2-5H-L except with slightly shorter fibers (see last picture). This switch achieves high transmission by physically moving fibers to align with other fibers. For multimode, losses are typically less 1.0dB @ 532nm (with air-gap).
The C012 requires a bipolar drive to switch between State 1 and State 2. When power is removed the switch returns to an “Off-State” where all fiber paths are routed to internal “dump fibers” within the package. The dump fibers are rated for up to +30dBm (1W) of laser power.
These were used to select one of a 532nm or 660nm laser, but can also be used in reverse to switch a single input to one of 2 outputs.
